The Iowa Department of Public Safety says alcohol and a high rate of speed were involved in the deadly accident in the early morning hours of March 26.

According to a release from the Iowa Department of Public Safety, Benjamin Edward Beary's blood-alcohol content was .223 percent, almost three times the legal limit in Iowa (.08). The investigation also revealed that he was traveling west on the eastbound lanes of Interstate 80 at more than 100 miles-per-hour when his vehicle hit a Des Moines police vehicle. Toxicology reports also indicated he had used marijuana recently.

Authorities say Beary entered the eastbound lanes of I-80 at the Grand Prairie Parkway off-ramp. The release states, "The investigators concluded that Mr. Beary's high level of impairment resulted in him driving the wrong way on the interstate."

The report states that Beary's vehicle had been traveling at 102 miles per hour for nearly a mile when it hit the Des Moines Police Department Ford Explorer that carried Officers Susan Farrell and Carlos Puente-Morales and prisoner Tosha Nicole Hyatt. All four people involved in the crash were killed.

The Iowa Department of Public Safety report also says the Ford Explorer was traveling 79 miles per hour, above the 70 miles per hour speed limit in that area.

Investigators do not believe the officers saw Beary's vehicle coming, due to a small incline in the interstate.
